Please assist us in keeping your child’s health record up to date by notifying the Health Office of any new physical conditions, treatments, or immunizations. Be assured that the confidentiality of your child’s medical issues is highly respected at the South Buffalo Charter School. To help us contact parents with a concern or in an emergency, please notify the school of any changes in your work or home phone numbers, as well as your home address.
Physical Exams and Dental Certificates
All newly admitted students and those entering grades K, 2, 4, and 7, are required to provide a current physical exam and dental certificate. Students that are involved with CSE/IEP/504 are requested to also have an annual physical. Physical forms are available in the main office and in the Health Office. Any student planning on participating in any extracurricular physical activities, i.e. basketball, track, baseball, walking club, etc., must have an updated physical on file in order for the student to participate.
During the school year, all students are screened in the following areas: vision, hearing, height, weight, scoliosis, body mass index (BMI) and blood pressure. During their screening, they will also receive preventative health education. Parents will be notified of any abnormal findings. Also, New York State schools are selected at random to participate in a Body Mass Index survey. If South Buffalo Charter School is selected for this survey, you will be notified and have the right to refuse to have your child’s BMI results submitted. Keep in mind; these results are recorded only as boys/girls in grades K, 2, 4, and 7 and not by name.
